Sheriff Department Records
Requests for police reports and other official documents of the Sheriff's Office can be made several ways.
The request should include your name, type of incident, date of incident, location of incident and case number if available. Click here for the request form.
The form may be submitted the following ways:
- Mail the request form to 100 W Broadway Ave, Suite 3, Montesano WA 98563.
- Fax the request form to 360-249-3288.
- E-mail the request form to firstname.lastname@example.org
- You can come into the office and fill out a request form.
